The American Society of Breast Surgery have produced a comprehensive review of the literature on Contralateral Mastectomy in patients with unilateral cancers (called prophylactic mastectomy in their papers). Mastitis and Mammary Abscess Management Audit (MAMMA) in the UK and Ireland
MAMMA describes the current practice in management of mastitis and breast abscess in UK and Ireland, with a specific focus on the rate of surgical intervention. From 69 participating hospitals, 1312 patient’s data were analysed (Aug 2020 to Aug 2021). Primary outcome measures included patient management pathway characteristics and treatment types (medical/radiological/surgical). MAMMA identified a high overall rate (21%) of breast abscess incision and drainage, and lower than anticipated rate of ultrasound-guided breast abscess aspiration (61%). Between individual units, significant variations were observed in the rate of incision and drainage (range 0–100%; P < 0.001) and needle aspiration (range 12.5–100%; P < 0.001). Overall requirement for inpatient treatment was 22.5%, with 72.9% of these patients requiring IV antibiotics. The odds of undergoing breast abscess incision and drainage or requiring inpatient treatments were significantly higher if patients presented at the weekend vs. weekday (P<0.023). Breast specialists reviewed 40.9% of all patients directly, despite the majority of study cohort (74.2%) presenting within weekday working hours.Read More chevron_right -

Following a literature review, the consensus statement from ABS is that, if excision of DCIS with a margin of 1mm or greater is achieved, routine re-excision is not warranted. This consensus statement can be found on the ABS guidance page under “Cancer Surgery”. The current recommendation by NICE is that further axillary treatment should be offered to women with micrometastases in sentinel lymph nodes. This differs from the recommendation by ABS, the European Society for Medical Oncology and the American Society for Clinical Oncology who advise against routine further axillary treatment in this group of patients. The ABS consensus statement on management of the malignant axilla can be found elsewhere in this section. The NICE guidance recommends that use of Adjuvant Online should be considered when planning adjuvant treatment for breast cancer patients. This is supported by ABS. NICE have updated their guidance on the management of early breast cancer to take into account developments since the 2009 iteration of this guidance. They recommend re-excision of breast tissue should be offered if DCIS or invasive cancer is present at the radial margins (tumour on ink). Re-excision should be considered if tumour is 0 to 2mm from radial margins. A similar conclusion is presented by ASCO. Following a literature review, the consensus statement from ABS is that, if excision of DCIS with a margin of 1mm or greater is achieved, routine re-excision is not warranted. This consensus statement can be found on the ABS guidance page under “Cancer Surgery”. Advice on breast reconstruction is now included in the guidance as is advice on extended endocrine therapy and lifestyle. Predict.nhs is recommended to aid decision making around adjuvant therapy and to estimate prognosis. There is no comment on tumour profiling tests to guide adjuvant chemotherapy decisions. ASCO have produced recommendations on the requirements for postmastectomy chest wall irradiation in patients with T1-2 tumours and 1-3 positive axillary nodes or patients with a positive sentinel node who do not undergo further axillary surgery. The European Society of breast specialists recommendations for the management of young women with breast cancer
EUSOMA define “young” as age <40 and “very young” as age <35y. Their guidance gives a very good overview of the evidence surrounding the treatment of breast cancer in young women. This includes difficult issues such as sexual dysfunction, fertility preservation and bone health. The EUSOMA guidance also recommends margins of 2mm of more for DCIS.
