Early and locally advanced breast cancer: diagnosis and treatment (CG80)

NICE recommends re-excision of breast tissue if radial margin of excision is less than 2mm. A similar conclusion is presented by ASCO. Following a literature review, the consensus statement from ABS is that, if excision of DCIS with a margin of 1mm or greater is achieved, routine re-excision is not warranted. This consensus statement can be found on the ABS guidance page under “Cancer Surgery”. The current recommendation by NICE is that further axillary treatment should be offered to women with micrometastases in sentinel lymph nodes. This differs from the recommendation by ABS, the European Society for Medical Oncology and the American Society for Clinical Oncology who advise against routine further axillary treatment in this group of patients. The NICE guidance recommends that use of Adjuvant Online should be considered when planning adjuvant treatment for breast cancer patients. This is supported by ABS.
